A general risk assessment template following the Health and Safety Executive's '5 Steps to Risk Assessment' approach. Identifies hazards, evaluates who may be harmed and how, records existing controls, and details further actions needed.
Incident Report Form
An incident report form for recording any accident, injury, near-miss, or unexpected event in accordance with CQC Regulation 12 and the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR). All incidents must be recorded promptly and accurately.

Mental Capacity Assessment Template
A Mental Capacity Act 2005 (MCA) compliant assessment template. The MCA states that a person must be assumed to have capacity unless it is established they lack it. This template follows the two-stage test: (1) Is there an impairment or disturbance of the mind? (2) Does this mean the person cannot make the specific decision?
Consent Forms
Consent forms based on the principles of the Mental Capacity Act 2005 and CQC Regulation 11 (Consent). Valid consent must be informed, voluntary, and given by a person with capacity. Where a person lacks capacity, any decision must be made in their best interests.
