Mental Capacity Assessment Template

A Mental Capacity Act 2005 (MCA) compliant assessment template. The MCA states that a person must be assumed to have capacity unless it is established they lack it. This template follows the two-stage test: (1) Is there an impairment or disturbance of the mind? (2) Does this mean the person cannot make the specific decision?
